It looks like replacing the fan is not trivial (requires a properly shaped tool, and thermal grease).

I just replaced the fan in a T43p after cleaning didn't fix the "rattle".

The replacement fan from Lenovo/IBM came with thermal grease already
applied so all I had to do was clean off the CPU. Turns out the most
difficult part of the replacement was getting the thermal pad loose
from the ATI FireGL chip - on the T43p the CPU fan cools both the CPU
(much smaller area) and the ATI GPU. The CPU is thermally bonded with
grease and the GPU with a heat conductive pad (that became effectively
glued onto the GPU). I had to use solvent to clean the GPU so the new
pad (attached to the fan assembly) would make good contact. The pre-
applied grease for the CPU worked fine. My machine now is mostly quiet
again - the old fan started to sound like an old Evenrude outboard :-)

Download the Hardware Maintenance Manual from Lenovo. It lists all the
steps in order. Wasn't that hard but I do have the correct tools and
static safe station at work.
