Naw, each tiddler is a separate file in a big pile (@node users, am I right?). It wouldn't make sense with "folders based on tags" since a tiddler can have multiple tags... (unless you then wanted it in multiple folders I guess... but then you'd have multiple instances of the same tiddler... or maybe they'd only behave as one if both folders/tags were active... anyway, no, that's not how it works.) Also, would it be possible to run filters/reports etc on the entire patient > database?
