If you don't want to use the tiddler.title as an UUID, you'll need to rewrite most of the core functions. But all this stuff has to be programmed. Since the underlaying system doesn't force you to do it, nobody will do it. It's as simple as that.
No, if the uuid a field, _nothing_ needs to change in tiddlywiki. Nothing at all.
