Are you sure this was produced by an MUA?  I thought exchange was the
server portion?

Looking through my maildirs, I see the various Outlook variants do
seem to use In-Reply-To or References.

``Internet Mail Service'' however does not.

Microsoft's inconsistency, incompetence and disregard for standards
never ceases to amaze me.
