This message was created by clicking Reply from Outlook 2002 (aka
Outlook XP) running against Exchange 2000. Take a look at the headers. I
don't think you'll find a References or In-Reply-To header. I think
Exchange uses Thread-Index and Thread-Topic headers. I don't think you
can tie those back to the original Message-ID.

I believe so.  There are no IRT or References fields in any of the
messages
I have received from any of my friends using Exchange (with Outlook).

I am almost completely Exchange ignorant (half intentionally), but what
my friends-in-the-know have told me is that Outlook can become part of
Exchange.  Maybe someone with a little more Exchange knowledge can step 
in here and clarify this.
