Searching on-line, I see that this problem occurred in early versions of TortoiseSVN/CVS as well.
I don't have any icon overlay handlers loaded. *Every single reference* to "Tortoise" has been removed from my computer and my registry. I've rebooted. I've deleted in my iconcache.db. I even installed and uninstalled Tortoise 0.4rc1 to see if that would make a difference. So far... I'm stuck with these unwanted icons. Ideas? Please?
