People with large libraries will be a lot better of using a separate
server, it's not worth struggling with the built-in server in the Touch
if you have a large library IMHO. The patch might help people which use
the USB port to connect USB sticks from friends since it makes them
accessible a bit faster than with the auto scanning enabled.

I'm guessing there is no search support unless you have done a complete
scan ? Would the system really be usable without search support in a
large library ?

IMHO, the built-in server on the Touch is a dead end, for it to work
reliably, we either need more powerful hardware or a completely
redesigned server software which uses a lot less resources. Among
these, my guess is that new powerful hardware is a more likely to
happen than a completely redesigned software.
