I only mention power issue because I had somewhat similar effect with a
GSM modem and a Pi-1B - work OK when plugin once Pi was up & running but
a reboot (no power off) - failed as Radio setup used to take a lot of
power (also easy to test and eliminate) . I don't expect modern BT LE to
take lots of power but older adaptor might - does BT on/off switch turn
antenna/radio circuit  on/off or whole device power  (check dmesg)?
