http://trac-hacks.org/wiki/SimpleTicketPlugin

It is a good place to start, however we want more types of tickets,
and also we want to ADD fields to the ticket, as well as HIDE. We
also want control of the HTML layout of the ticket form.

Any ideas where to find a developer to build such a system? We'd be
happy to contribute back to open source.
