The line item control number is generated by the provider's claim system.
Since these systems vary by provider, I doubt there is any consistent rule
as to how the number is formated. The theory is that the provider's claim
system will use this contol number as part of an automated posting of their
electronic remittance. The 835 guide states that if submitted on the
claim, it must be returned on the remittance if payments and/or adjustments
are reported at the line level.
Therefore, if a payer truncates this control number and only returns the
truncated number on the 835 remittance, the provider would not be able to
use the line item control number as part of the automated posting. Since
the HIPAA guides allow for up to 30 characters, and requires that it be
returned on the 835, this is what must be accomodated. The only way I know
of to change this would be through a DSMO request to limit the size, but I
do not belive this woulb be acted upon in time for the first go round.

In the 837 professional IG it states that the line item control number (pg
472) can be 1 to 30 bytes. Is there any scheme that will be used in this
field, we do not have enough space on our current screens to allow an extra
30 bytes per line and so we planed on truncating in our processing systems
.
In order to still use the field to uniquely ID the lines within the
adjudication system we are trying to determine if the last 10 or the first
10 would be more beneficial in identification. If anyone currently works
with this field or has any thoughts around how it will be used I would
appreciate any comments.
