Chris,
The 2440 loop became a part of the X12 standard as of version 4, release 1,
subrelease 1 (aka 4011).  I've checked and it is still in the standard as of
4050 (and is also in the current draft of the Implementation Guide for the
837 Professional Claim).

Every October, X12 publishes a new version/release, such as 3050, 4010,
4020, 4050, etc.  In February and June, X12 publishes subreleases, such as
3051, 4011, 4012, etc.  X12 published the standards for version 4050 as of
October, 2001, and will publish subreleases 4051 and 4052 in February and
June, 2002, respectively.

All implementation guides are based on some version of the X12 standard.
The HIPAA guides published in May, 2000, were based on version 4010 (with
the exception of the 2440 loop).

I would expressed Anthony's message a little differently, but the end result
is the same.
HIPAA is based on the Implementation Guide 004010X098, published in May,
2000.  By that time, X12 version 4011 had long been published.  (It was
originally published in February, 1997.)  That guide (X098 is for the 837
Professional Claim) is based on version 4010 (not 4011), but contains an
exception to overcome an otherwise insurmountable problem.  See Usage Note
Four for the segment notes to the FRM segment (page 567).  The note says,
"4.     Loop 2440 was approved by ASC X12 in the version 004011 Data Dictionary
but is included in this guide to provide [a] standard way to report DMERC
claims within the HIPAA Implementation time frame.  It is recommended that
entities who have a need to submit or receive DMERC claims customize their
004010 translator map to allow this loop."

This is another surprise because I didn't know there WAS a "4011"
version.  I assume that the 2440 loop will be retained in the 4050... would
this be a safe assumption?
