This is an issue for the 276. The 837 IG doesn't mandate the CLM01 be
unique.  It is under the provider's control.  It suggests a claim number
(unique?) or patient ID.  If it were a unique claim number the 276 could use
a TRN02 to match.  The patient ID is better placed in the 2300 REF02 when
REF01=1A but that's not in the Dental so back to the CLM01 and not being
unique.  

Sorry, don't have the anwer.  I'm just asking whoever answers you general
question also tie it to the 276 too.  provider-patient-date-amt.... seems to
be too complex of a combination.

How can one identify claim uniquely? According to IG, I infer CLM01 can
be used. But the question is, is this field unique through out, i.e
