Since CLM-01 is not under the control of the payer, you can not count on its uniqueness. But still I would bet that it is unique. Of course it is not unique across multiple providers but the combination of provider id and CLM-01 is reasonably unique. If you still can't come up with a unique match, then so be it. You tried. It should be a very remote case. The matter that CLM-01 should be reflected in TRN-02 in the 276 is not really as clean as I would hope. If I have a claim with multiple payers involved, I can only identify the 276/277 communication by the claim number. If multiple payers send back their 277s, I have to add additional logic to clearly identify them back to my request. TRN-03 and TRN-04 are not used per IG. It would be better to allow for these further qualifiers.

The 837 IG doesn't mandate the CLM01 be unique. It is under the provider's control. It suggests a claim number (unique?) or patient ID.
