I was notified by NTTA. NTT had just been in a meeting with JATE regarding changes to modem JATE forms. they contacted me to let me know that they will be sending me new form information. I'll go back to my source to re-verify and let everyone know. I wouldn't count on it being a rumor since NTT is not known for starting rumors.
