Just remember, the nation's telephone system was not designed, it evolved.
Therefore, there are no absolutes.  There probably still are Strowger
switches around (Lord knows, maybe even a Panel Office) and some chicken
farms out of Tonopah NV, may still be served on barbed wire and ground.  
What you won't read much about in the recommended literature is the
impairments introduced by building wiring.  As more buildings are being
wired by contractors who had the lowest bid, you'll find more large
residential buildings (especially) that have bel wire for communications. 
Try and run a high speed data service over it to the typical work-from-home
user.
