Backup and HARD reset. If it keeps doin' it then it's a hardware issue and the phone has to be replaced.
If not, it's a software issue, and you'll have to dig a little to find the offending application. Check the interval between the next 5 "wake ups". If it's "to the second" (very similar times), then it's very likely a software issue.
