I was thinking it might be good to send the response directly for templates
which end up with a null layout.  Haven't worked out all the details though.
And I was wondering if it might be confusing.  If a template has a layout,
it does not include such things as <html>, <head>, and <body>.  However, if
no layout is matched, it is expected that the template includes these
elements itself.
