Hi Douglas,
  Right, I've looked at your images - and I see you're landing at the initramfs 
prompt.  Can you include (from a successful boot) a cat of /proc/mdstat and cat 
/proc/scsi/scsi /proc/partitions and /etc/fstab?

Also if you land at that initramfs prompt you might try capturing the
contents of each of those /proc files.

What I've seen before on systems with complex md setups is that it can
hit ordering issues - make sure the RAID never uses /dev/sda or the
like, try to always use stuff based on UUID or label or path because the
order of initialisation is random.
