I just checked the hardware. The card is a mini PCIe card, it is firmly
seated in its slot and held in place with a screw, basically no way it
could inadavertently come loose. That pretty much rules out hardware
issues, apart from bad solder joints or broken circuit boards.

In conclusion, I currently consider firmware the most likely source of
the problem.
