Seems quite clear on our bill. As you know, they bill in advance. When the 1/5/2013 price change took effect, on our next quarterly bill (1/7) we were credited the original charge for the period 1/5 - 30/6 and billed again at the new price, with the net effect that the price reduction was retroactive to 1/5/2013.
Yes it applied to 5 year term circuits as well, at least in that case. It will depend on what is shown in the price list. 5 year term EAD pricing is always listed separately in each price list section.
