If the runlevel change didn't stick, it sounds like maybe Yast doesn't like (or doesn't understand) the init script.
You may want to open it in an editor to make sure it doesn't have any bad/strange characters in it (especially if you may have originally edited the file in Windows and then transferred it to Linux).
