Hi Jake,
In short, if someone knows what IP address you're on, yes. There
are in fact, some free services which offer 'dynamic IP address services',
which allow you to run your own server, utilising some sort of software
which your computer calls their service & their DNS updates & you can have
your website running each time your IP changes. Hope that's of some help
