1. What tables you use in MySql?
MySam or InnoDB?
YOu 'must' use InneDB tables, because they are transactional save, mySams 
aren't.
So there its is very fast achieved, that a table/data gets corrupt.

2. How many instances do you have running?
Is it possible, that two instances are trying to use the sam persistence 
manager?

3. Try out derby just for comparison.
Do you have there the same problem?
If not that would point to:
- Wrong persistance manager configuration (as Tobias wrote, like shareing the 
same repo folder)
- Problem with this mySQL instance as using wrong tables (mySam).
