Thanks David, that's really helped. I've built a new NPS server and configured the Azure Extension on it, configured the policies as per your attachment. I repointed Guacamole to use the new NPS server name, but unfortunately I still can't login to Guacamole with AD creds. I've tried with several test users and AD/Guac groups, but I can still only login with the password stored in MySQL.
I've checked the Windows event log for any NPS/Radius error but there are none, I've also enabled debug logging in /etc/guacamole/logback.xml but I can't see anything useful.
