Hi PlayerOne, on the Windows NPS Server's Firewall Advanced Security - Inbound Rules, have you added a new rule: Port, UDP, specific local port 1812 ? If your guacamole server lives in a DMZ and your NPS is on another segment, then you may need to open the rule, "Advanced" tab, and "Allow edge traversal". The default rule installed when the NPS is installed doesn't appear to work for some reason.
[I will add the above to the first Post.] If that doesn't work, have you tried, for example, a VPN connection with your firewall as the VPN endpoint (ie not using the Guacamole Radius Authentication Provider) ? The Azure MFA NPS extension is very good at creating spurious event log messages, so assuming you have it installed, success and failure will be quite visible in the "Computer Management" MMC.
