You did do all the following?

- patch the file
- make clean (in the php/midgard directory)
- make (in the php directory)
- make install
- stop apache
- start apache

The latter cannot be a restart. Apache doesn't reload modules reliably
when the't been recompiled.

Not the fact that we're not seeing debug info, but the url could be a
Asgard issue. If the above steps have been performed and there's no
debug info in the error log the problem isn't directly tied to
attachments and I'd need to know what Asgard does more when
attachments are submitted, hence the call for our Asgard expert.
