In the guide, it says:

"Adjusting the Printer for the Printout 
Printing envelopes depends very much on the type of printer used. Some
printers require a special cassette, while others have a multi-purpose
tray that can feed envelopes. If in doubt, consult the manual for the
printer. 

Specify any print options in the Page Style: Envelope dialogue. To open
this dialogue, click an empty spot on the envelope, open the context
menu, and select Page.... Then select the Page tab. Define the desired
settings under Paper Format. 

Check File > Print ... > Properties to ensure that the printer knows the
size of the envelope being used. When this step is skipped, printing of
an envelope becomes unpredictable.
"

It is this last line that seems to be missed consistently. Is this the
problem?

Envelope orientation should be taken care of in OOo on the Format tab.
Is this not the case?

Like the manual excerpt above says, behaviour is unpredictable if the
printer properties are not set correctly. This is true in 1.1.x and
2.0bc.

Perhaps the printer properties line should be in colour or marked as a
warning. 

The other thing that could be done, if not done already, is to create an
RFE to have the OOo settings also set the printer properties. This would
certainly be a worthy RFE and would eliminate some confusion.
