I am indeed. When I first open the insert index/table dialog, I get five tabs: Index/Table | Entries | Styles | Columns | Background
Under Index/Table, that's where I set the title of the TOC, the type, and select the "create from". I've tried three ways with the create from: Outline - this is the default Additional Styles - this was on the suggestion of the FAQ on the site. Outline AND additional styles - this was a last-ditch effort to make something work. All three provide the same results: only Chapter 2 sets up. I'm not really sure what to do with the Entries tab, but I've tried just about every setting under there, to no avail. Styles has two columns - Levels and Paragraph styles. Under levels, it shows "Title" and Levels 1 - 10. Under Paragraph Styles, it has all the others, including Rage Heading 1 (the name of my custom style). I've trying selecting that, but the arrow between the two columns is greyed out. I assume if it were not, I could add it to the Levels list. Columns and Background are both formatting, and I don't need any of that. To create the Chapter Heading, I went under Tools => Outline and Numbering For Level one, I changed the Paragraph Style to Rage Heading 1, Number to Number, and under Separator, I put "Chapter" in the before box. This makes the Chapter Numbers appear as "Chapter 1" and number automatically. Under the Position tab, it's set to Level 1 This was how the FAQ indicated it should be done. I've checked all my headings, undone them, restarted, and always, I get the same result: a TOC with only the title and Chapter 2 listed. The chapter headings do appear in the scroll bar at the side when paging down, and also in the footer I created with a field (added from Insert => Fields => Other => Chapter: Chapter Name Thanks for your help, Barbara. Heather, are you still having problems with the TOC? If so, it might help if you let us know what you have in the various tabs of the "Insert Index/Table" dialog -- there's a lot going on in there.
