You started out correctly. Insert > Header or Insert > Footer. In
either case, a context menu pops up with the word Default. You need
to click Default in insert the header or footer. Default is the only
listing in the menu because that is the only page style you are
using. There are times when I use a different page style. If I want
to insert a header (or footer) on that page I use Insert > Header.
Both Default and the name of my page style appear in the menu.
