I can see it on my system. The dialogue looks like this, from left to right
and up to down:

Search path buttons
A Places-listbox - A Files/folders-listbox
An Add-button - A Delete-button - A format selection thing
File type selection - "Type" selection
A Preview-box
A Link-box
Cancel-button - Open-button

My OS is Ubuntu 7.10, OpenOffice.org 2.3.0.
