I choose to put the washer in-between the nuts for one main reason......It's
much easier to tighten everything because the nuts are separated. You can also
use a thicker wrench to hold the bottom nut when tightening the top nut.
Mechanically it really doesn't matter weather the washer goes on top or
in-between the nuts. It is only there to keep the proper dimension.
