Yes we are still doing the initial baseline circumference in order to have a 
reference point if needed, however we have discontinued the daily circumference 
measurements.

We still do a baseline measurement at the insertion site and document the 
circumference at the insertion site (which is variable of course).  We never 
did a specific place measurement.  Our documentation has always included 
where we measured.
We have found a few patients with "swelling" on the PICC arm and we could 
document there was no difference from our original measurement.  We have 
also found very few that have had an increase of 1-2 cm. that did have a 
thrombus in the arm, detected early, treated early.
We don't require staff to measure regularly.  But if the question comes up, 
we are comfortable having the baseline measurement, when called about 
assessing the site again.
