Just a little anectdote to support this, one of our interventionalists reported clearing a 5 year long clotted ptfe dialysis graft with TPA. I don't know the details but I know he doesn't exagerate.
So yes always give it a try, it just may take more patience.

According to an analysis of the COOL clinical trial data, there was a 91%
success rate when Cathflo was instilled in catheters with thrombotic
occlusions < 24 hours old. For clots formed within 24 hours up to 2 weeks,
the rate of success was still good but a bit lower at 87% (I think that is
the right %). It drops a bit more after 2 weeks.
The take-home message is to still always try Cathflo no matter how old the
occlusion, but that the sooner it is used the greater the chance of success.
