This is a good question, I'll be interested in seeing how others respond to it. Myself, I don't change patterns very often. Usually I choose my initial pattern depending on what I can see going on around me, if I see fish rising (and can tell what they're rising to) I'll go dry. If not, I'll tie on a searching pattern such as a PTN or hare's ear.
A friend who I fish with regularly changes flies about every 15 minutes if he isn't getting any action. I kid him that he has the fly in his fingers more than in the water. If I'm not getting any action I tend to fish the same fly more intensely, paying more attention to how I'm presenting it and so on, rather than swapping around a lot. We both tend to catch about the same number of fish on average. We will also split the difference when we go out, for instance, I'll fish wet and he"ll fish dry and see who gets any strikes.
It would be nice to see other VFB'ers takes on this.

Personally, I'll change patterns or tandem rigs every hour to hour and a
half if nothing likes what I have to offer, trying to use each offering in a
couple different types of water to make sure I'm fishing somewhere that
seems promising. When I change it will usually be either to something that
is a general imitation (PT, hare's ear, caddis dry or midge) or to something
that looks like what comes up attached to rocks, is fluttering around the
water or ends up in my seine net.
