FOLLOW UP: I found an upgrade that updated the core engine from 4.0 to 4.03. Everything now works as it should. There must be some incompatibility between 4.0 and the IIfx/8.1 combo. There is very little information on 4.0 out there on the web so I'm not sure if this was an issue that was known at the time or not.
