Well, despite all my efforts to use one or two fingers as appropriate; a lapse of attention means a third finger touches without my realising, and also wonder why speech goes off. So worth doing the 3finger double tap to see if it comes back on. I also tend to try a 2finger flick up to get voiceover talking if it's doing it at all, then crank up the volume in the hopes that it was only minimised.
After that, I'm not sure what to suggest.
