My guess is that out of the box no screen reader is going to play all that well with iTunes, it is doable, but takes a lot of time. One of the best interfaces I have seen to work is Jaws and Leasey, however, I had trouble paying for Leasey last summer, PayPal and I are not on the best of terms, and I avoid it like the plague. There were some features in Leasey I'd never need or use but the iTunes experience for the few days I had the program was really one of the best.
