My guess is that out of the box no screen reader is going to play all that well 
with iTunes, it is doable, but takes a lot of time.  
One of the best interfaces I have seen to work is Jaws and Leasey, however, I 
had trouble paying for Leasey last summer, PayPal and I are not on the best of 
terms, and I avoid it like the plague.  There were some features in Leasey I'd 
never need or use but the iTunes experience for the few days I had the program 
was really one of the best.

I've been using iTunes with Jaws for years with greater and lesser
degrees of success depending on the versions of both applications. The
BlindTunes scripts are helpful but not essential. And like someone
else said, I prefer to do as much as possible on the iPhone though I
an do backups and restores with iTunes.
