Do you have "containers" in your rotor? When I added containers to my rotor & 
had containers selected in my rotor, I was able to reproduce your problem with 
iAssociates. 

If you do have containers in your rotor, try removing it from the rotor by 
going to settings, general, accessibility, voiceover & swiping down to rotor & 
unselect containers. Try opening iAssociates & see if the problem still exists.
