Hi, I'd suggest folks write to apple.com/feedback and under iPhone describe concerns they may have about this. I have just done this, and I think if they hear from more folks on this issue, the more they would likely look into it.
I emphasized the problems of a sighted emergency worker looking at the iPhone of a blind person with voiceover active and screen curtain enabled. Hopefully I provided enough clarity, but if others could follow on this (I understand they look at each suggestion daily) that maybe a solution could be implemented. If you do turn this on, you first have to wake up your phone, then double tap on the unlock button, then when you are on the screen with the keypad where you enter your Pin you have a button called "Emergency Calls" at the bottom left, double tap on that and on the next screen in the exact same location at the bottom left will be the "Medical Id" button, tap it and all the information including emergency contacts with their relationship to you, name and phone numbers will be displayed. Anybody who has your phone can call any of the contacts listed here without having to unlock the phone simply by tapping on their name.
