It does not work quite like this for me, and I think it could be because all of 
my contacts have two or more numbers specified for them.  I believe, when this 
is the case, when you double tap on the contact, you are then forced to choose 
which number is to be listed.

Once I choose a number, I then get the relationship screen (but it took me a 
while to realize this).

Just to emphasize this one more time, when I do this I am in the Health app, I 
double tap on "Medical Id" at the bottom right, now that my medical Id is set 
up I double tap on "Edit" at the very bottom.
Now when I'm where I can edit the information I swipe past the 3 emergency 
contacts I have already set up, my wife, my brother and my mother, then I am on 
where it says "Add Emergency Contact", Voiceover does not identify this as a 
button, but I double tap on this and my contacts open up.
I now find the contact I want and double tap on it which really is the only 
thing you can do, there is no action rotor or "More Info".
Once I double tap on it I get a screen with a cancel button at the top left, if 
I swipe right from there or touch at the top of the screen Voiceover says 
"Relationship" which is a heading. Underneath this is a list, I have the 
following choices:
Mother
Father
Parent
Brother
Sister
Child
Friend
Spouse
Partner
Assistant
Manager
Other
Roommate
Doctor
Emergency
As soon as I double tap any of these I am back in the main editing screen for 
my medical Id and the person I selected is now listed with the label I chose. 
The name when it is spoken is followed by "Text Field", but double tapping on 
it doesn't do anything, but there is a delete rotor option if you want to 
delete one of them.
If this is not how it works for you then I have no idea why it is different.

I still cannot get this to work either.

I see the button labeled "add emergency contact" which opens up your list of 
contacts, but exactly what do you do in this list to choose a contact?  

I have been trying a single finger double tap, but that just opens up all the 
options for the contact (to call, to message, to edit, etc), but I think my 
problem may be that I don't know the proper way to just "select" the contact so 
that I can close the list of contacts.  It's probably not to double tap the 
contact?

Yes, and that's how I thought it was supposed to work, but then someone took 
the phone from me and they did it ... and so I am not sure why they told me 
they had to be named with ICE.

No, I simply selected my wife, mother and brother, gave them the appropriate 
label (Spouse, Mother, Brother) and they show up just fine.

Yes, this sounds right, but for some reason I could only get my emergency 
contacts to show up on the lock screen, even when doing it via the Health app, 
only when I named them beginning with the letters "ICE".  I tried to use 
existing contacts and it would not show them.  Did anyone else run into this?

Hi,

If you go to the Health app, tap on Medical Id at the bottom right you can set 
up all sorts of important information including any medical conditions, 
medications, alergies, your height, weight, whether you are an organ doner and 
who your emergency contacts are. You can also specify at the top if this 
information should be available on the lock screen.
