Also, if your Dad set up Face Id that would probably make it much easier and there would be a lot less chance for this to happen. He would still have to put in his passcode from time to time, but not as often. Also, he must have really tried many many times for the phone to lock like that. I’d make sure the triple click option to enable Voiceover is turned on so you can more easily help him and tell him that if he is trying to unlock his phone with passcode and he tried 4 or 5 times and it doesn’t work to not keep trying although I believe it takes at least 10 attempts for the phone to lock down.

Ran the DFU update again with the Apple Accessibility folks. We finally got it restored. I have to say, I hate this phone! I know your supposed to run one finger up the edge to go home but it only works part of the time fore me. Even dad who is sighted misses the home button. He says there are too many gestures.
