If you heard VoiceOver say speech off, you accidentally did a three finger double tap. Been there, done that, even though I would have sworn that I only used two fingers.
But, the later iOS builds are way more responsive and it truly seems like sometimes just a single skin cell can touch the screen and have it react. That extra finger just has to barely graze the surface to have this happen. If you hear VoiceOver say Speech Off, that is the result of a three finger double tap. That is the only gesture that will do that. As Holly said, just do it again to toggle it back on.
