You may have thought you did a 2-finger double tap, but you may have touched the screen elsewhere and the phone thought you did a 3-finger double tap. Another possibility is that your screen is dirty and that can sometimes result in unpredictable results when you do gestures.

That extra finger just has to barely graze the surface to have this happen. If you hear VoiceOver say Speech Off, that is the result of a three finger double tap.
