First of all, it sounds to me like maybe something went not quite right with 
your update because I have not heard of any issues with people being able to 
power off their phone in the usual way, e.g. press volume up, press volume down 
and then press and hold the side button for about a second or two until the 
power off screen comes up.
You mention the "double tap to power off", "emergency call" and "cancel" 
screen. Just FYI, there is no emergency call on that screen and hasn't been for 
some time.
Also, you say to do a hard reset you press volume up and volume down and then 
you hold volume down and the side button at the same time. I assume it is no 
surprise that this won't force a hard reset because it is the wrong button 
press combination. Yes, you press volume up followed by volume down, but then 
all you do is press and hold the side button for maybe a 12 to 15 count 
depending on how fast you count. 
Also, in iOS 16 you can simply tell Siri "Reboot my phone", you will be asked 
"Just to confirm, you want to restart this device", then you say "Yes" and the 
phone reboots, nothing to press or do, just wait until the lockscreen comes 
back up.
I have an iPhone 11 and while I am on the iOS 16.1 Beta after using the 
official iOS 16 and 16.02 release for a short time, I don't recall the app 
switcher issue to be present for every app, e.g. if you had 10 apps open if I 
remember correctly only the last one was listed twice.
Anyways, I would highly recommend you maybe start with a simple reboot, if that 
doesn't help try Settings > General > Reset and select "Reset Network Settings. 
This will require you to re-enter your Wi-Fi password and name you r phone 
again in Settings > General > About, but it can often have some quite 
beneficial results.
If that also doesn't help make sure you have a backup and completely reset  
your phone.

Yes, definitely having issues with the app switcher.  I was forced to update to 
16.0.3 on my iPhone 8, in order to use my new airPods Pro.  Many Things have 
been challenging ever since.  After entering the app switcher, it is necessary 
to listen for "active" after the app name.  If you just hear the name of the 
app, keep swiping; if you have several apps open, this can be frustrating and 
time consuming.  

I am so upset with what's happening with the notification center.  Until 
speaking with Apple Accessibility, I was unable to open notifications.  I could 
clear notifications but not open them.  I read somewhere that "list" should be 
selected in the notification settings, so I did that; there was no change.  I 
learned about a work around, and am grateful for it, but again it is so 
frustrating.  I have to double tap on a notification, quickly swipe left to 
where I hear open, and then quickly double tap.  And, I am not kidding about 
the need to move quickly.  This method has been working pretty well until there 
was a reminder in the notification center; notifications could only be removed 
after I cleared the reminder.

I don't understand why, but I am unable to do a hard reset after the update.  
After tapping the volume buttons (up and then down) and then holding in on the 
down volume/side button for well over 30 seconds until my hands give out, the 
screen with "double tap to power off", "emergency call" and "cancel" remains 
and does not shut down/reset.  I wanted to try the forced reset to try and 
remedy the crazy behaviors; turns out, I don't think that would have helped as 
others are experiencing the same issues.

I'm wondering if my issues are related to my using the iPhone 8?  It would be 
interesting to know if similar issues occur on newer phones.  I'm giving 
serious thought to getting a 14 Pro, but I hear things aren't all that great 
for users of that model and/or IOS 16.0.3.  I am nervous about the combination 
of dealing with learning to function without the home button and at the same 
time dealing with IOS 16 issues.  As part of my research and/or decision making 
process as to whether to switch to the 14 Pro, while at the Apple Store the 
other day, I tried to open the app switcher and control center.  I didn't get 
to play around for more than 5 minutes, but never had success using the 
techniques described on this list.  An employee said it may be due to the 
device only being a model and not a fully functioning phone.  Another employee 
said they have 2 visually impaired employees on staff, but both weren't in that 
day.  He remarked that one of those employees mentioned that there is a problem 
with accessing the app center via voiceover when using IOS 16, and that perhaps 
that is why I didn't have success.

Hello, all I noticed that when I open the app switcher there seems to be two 
occurrences of the app. For example, if I had just mail open, I would hear the 
word mail and then if I swipe to the right, it would say mail button and then I 
can flick up and close it if I do not swipe to the second Word I can’t close 
it. I wonder if anyone else is having this issue? I’m not sure if I phrased it 
very well, but it seems bizarre to me.
