Paula,  I'm sorry if a factory reset is the only thing to get your iPhone talking again. Did you ever call Apple Accessibility and ask them to connect to your device to help with this issue. One time they helped me when VoiceOver volume got set to 0 when I was switching between the iPhone's external speaker and a connected Bluetooth speaker.
