Did you set it up yourself using VoiceOver? If so, the require attention should be off, which helps.
You want to hold the pone about 10 to 12 inches from your face with your nose pointed at the middle of the screen. Good lighting in the room may help. I've been using Face ID for a few years, and I too find sometimes it doesn't work and I have to use my passcode but that is usually operator error. But, if the pone rebooted, or it thinks it has, it will require your passcode.
