Hi Kim,

Since Voiceover doesn't read the names of the ringtones in the "Manage
RingTones" section, I suggest you create one at a time. If you do that and
then go into the Manage RingTones area, you hear the following:

At the very top left is "Back, button"
Swipe right once and you hear "Manage RingTones, heading"
Swipe right again and you hear "Edit, button".
Swipe right again and you get "RingTone Files, heading".
Now, if you swipe right one more time you hear the Voiceover sound you hear
when you swipe, but nothing is spoken.
This means you are now on your Ringtone. If you double tap the following
three options appear further down the screen:

Rename
Email
Play

Of course you can tap Play to see what RingTone you are on if you have more
than 1. If you select email the mail app opens, your file is attached and
all you have to do is type in your email and tap "Send". This will email the
file to you, you could then simply save the attachment into the RingTones
folder in your iTunes Media folder, open iTunes, make sure it's selected and
sync. If you have your Tones set to "Sync All Tones" you don't have to worry
about selecting it, just save it and sync.

I got this app and it is very easy to use. I was able to create a ringtone
in seconds from a track in my iTunes library on my phone. Where I
encountered a problem was when I tried to locate the ringtone I had just
created. It wasn't with the other custom ringtones on my phone. And the
manage ringtones area of the app seems very VO unfriendly as does the help
file. 

This could be a great little app with some tweaking for accessibility.
