Sorry Paul, speak selected text is in accessibility, not VoiceOver. This is 
actually quite important. Speak selected text is for people who are not running 
VoiceOver. When speak selected text is turned on, the iOS device speaks the 
text which a sighted user has highlighted. Recently, a sighted friend 
demonstrated this to me. He opened his iBooks, opened a book, highlighted a 
large section of the book, and his iPhone started reading the book to him. He 
does this every day on his way to and from work. Note: in general, sighted 
people never open VoiceOver in accessibility. When they attempt to, a warning 
pops up stating VoiceOver will change how gestures work. So, features that are 
outside the VoiceOver section under accessibility are for more than just blind 
users.

Speak in lock screen tells VO that it is acceptable to provide a spoken alert 
when the screen is locked. Notifications allows you to select the alert type 
you prefer for each app. Badges places an announcement next to the icon, such 
as so many new items. Banners pops up a small banner across the top of the 
screen with an announcement from an app. I have messages set to banners, so a 
text message appears briefly across the top of my iPhone and is immediately 
read by VO. The banner disappears after a few seconds. Alerts opens a pop-up 
window with the alert in it. You must interact with something to close the 
window. For instance, my phone alerts when there is an incoming call. I choose 
answer or ignore. If I choose answer, the phone screen pops up and the phone is 
answered. If I choose ignore, the screen returns to where I was.

You can select your choice from what the app makes available in settings, 
notifications.
