This just happened to me yesterday. I had done a triple click to turn
off VO so I could tap on something in NetFlix, and when I tried to use
triple click home to turn VO back on, the touch screen stopped working
and I wasn't getting any output from VO.
I tried powering off the iPhone, but neither I nor my sighted wife could
slide or double tap on the screen to power it off. I did a hard reset,
and when it came back up, the triple click to turn VO wasn't working. I
had to get my wife to go in and turn on VoiceOver, although I suppose I
could have done this through iTunes on my PC.
If I've ever had to do a hard reset before on my iPhone, I don't recall it.

I’ve found the same trouble, and ‘ve had to get someone with sight to put the
voice over back again, and also, it doesn’t come back to the same voice you
had, before the hard reset.

When resetting my iPhone using the hard reset (pressing the top button and the
home button at the same time for a few seconds), many times the voice over
function will not come back on. I will then need sighted assistance to go
through the settings menu to turn voice over back on.
Is there any way to avoid this, or is this simply a bug in the operating system?
