I'm not aware of such an app, but that doesn't necessarily mean there isn't one. And to answer your other question, I believe Pages does do things like footnotes, but how accessible they are I wouldn't know, as pages is not completely accessible, or at least it wasn't last time I played with it which was probably about six months ago.
